After submitting this form, be sure to request the following from faculty members:

Mentor letter — must include an explicit statement of support for the trainee’s participation in all ADRD Training Program activities.
UF faculty letter — from a committee member, course director, or collaborator.
Department Chair letter — affirming commitment to provide financial support for degree completion after ADRD funding ends.
